The health risk assessment program was developed by the State of Rhode Island Department of Health to make you aware of your risk factors and how you can eliminate them so you will have a richer, fuller life. The first step is to complete the health risk assessment questionnaire by going to www.riskassessment.com. The second step is to review the information provided as a result of the questionnaire and develop a plan of action to correct or reduce the risks that have been detected. There is also a Personality Analysis that is designed to help you better understand your own personality.
